Remove unused exn and example code

* Remove exn:fail:cookie, since I’d rewritten the library code to
avoid using it.
 * Correct docs for cookie-header->alist, to reflect not raising the
exn.
 * Remove needless illustration of date from clear-cookie-header
example.
